What is cellulite and what causes cellulite?

Cellulite is a very common condition, with over 90 percent of all women affected. There are 3 components that lead to the appearance of dimpled skin called cellulite. As we age, our fat cells get larger, skin gets thinner, and the fibrous bands under the skin tighten irregularly. These fibrous bands called septa stiffen or shrink over time causing cellulite butt dimples and leg dimples.  Unfortunately, weight loss, massage, and topical creams do not always address the problem of a true cellulite dimple. An individual dimple can have 7-10 individual septa that create the one dimple! There are no exercises to get rid of cellulite either. AVELI, a unique device, allows Dr. Laurie Casas to precisely identify, release each septa and then verify the dimple on the butt or thigh is completely released and the skin is smooth. There are various treatments on how to remove cellulite, although no other procedure allows for this kind of precision and verification.

 What to expect

After a thorough consultation with our nationally recognized female board-certified plastic surgeon, Dr. Laurie Casas will first determine the best comprehensive treatment on how to get rid of cellulite on butt or thighs. She will then differentiate and educate you on whether the areas have skin laxity, focal fat deposits, or cellulite dimples. Dr. Casas can provide and perform treatment options for all three separate issues that create cellulite looking skin. If you have and you want help in getting rid of cellulite dimples of the butt and thighs, she will offer AVELI. For severe skin laxity, she will review the treatment protocols to improve thigh and butt skin appearance and skin laxity. If you also have focal fat deposits, Dr. Casas will review timing and treatment options to treat the fat. 

AVELI treatment protocol and procedure 

To reduce bruising and to accelerate the healing process, Dr. Casas will instruct you on dietary restrictions and start you on topical treatment with Alastin Reform and Repair. Patients typically drive to and from their AVELI treatment. Before the procedure is started, a local anesthetic is injected into the area being treated. Dr. Casas then uses the AVELI device which she passes under the anesthetized skin to identify each septa within each dimple. Dr Casas has found as many as 20 septa in each dimple. Once every septa is released, she will verify that the dimple is completely released.  Following the release of every dimple, Dr. Casas will cover the area with an absorbent dressing and cushioned pads, along with a tight workout legging to minimize bruising and swelling post procedure. Patients may return to normal activities immediately after the treatment. Dr. Casas asks that you avoid vigorous lower body exercise, like running for 10-14 days. Patients describe moderate discomfort when sitting on the treated areas for 2-3 days then minimal discomfort for up to 1-2 weeks.

Aveli Clinical Results

AVELI was studied on 74 patients across 9 clinical sites. A total of 95.6% of patients were responders on the Global Aesthetic Improvement Scale. 80% of the patients were either much improved or very much improved. No unanticipated, serious, or severe adverse events occurred in the study.

What to expect post-procedure

Within 1-2 days, minimal downtime is to be expected, you may experience mild discomfort or pain. Clear or pink fluid may leak from the insertion points for 24-48 hours and resolve within a week, typically. 

Within 14 days, wear compression clothing as much as possible to help reduce swelling and bruising. You may engage in light physical activity. Strenuous activity may increase side effects. As swelling and bruising subsides, you should start seeing smoother skin. 

Within 30 days, avoid extended sun exposure and use sunscreen over the insertion points. Most bruising is fully resolved within 30 days of the procedure. By one month you should see a reduction in the appearance of cellulite. 

Within 90 days, after photos will be taken. Areas of firmness are expected, and you may perform firm massage until resolved. A minority of patients will experience area of numbness that typically resolves within the first 60 days. Results should be visible by 90 days

Aveli FAQ

Aveli cellulite treatment cost is a common question we get asked. The total cost of the treatment is based on the number of areas treated and how much cellulite there is. Cost can vary by provider, but a general cost runs between $3,000 and $5,000. The most common areas treated include the thighs, hips, and buttocks since these are the locations that are most likely to develop cellulite. It's important to discuss this prior to treatment, so you know what to expect. Please note that this elective treatment is not covered by insurance. Check with your provider to understand financing or payment options that may be available. 
It always helps to put minds at ease by clearly understanding what to expect during each treatment. Fortunately, Avelie is a straightforward procedure. Here's how it works:  Cellulite from subdermal bands of connective tissue, also called septa, puts pressure on the skin's surface. The result is an uneven, lumpy appearance that looks somewhat like cottage cheese.  Of course, uneven skin can certainly have an impact on confidence, so patients are interested in treatments that can address these concerns quickly. Aveli works to cut those bands of connective tissue so that the skin can once again have a smooth surface. It's a fast and virtually pain-free experience that leaves the skin looking and feeling its best.  During the treatment, the doctor uses a handheld device that has both a light and a hook. The light works to guide the device to the connective tissue, and the hook releases a small blade that severs the septa. The result is no more dimpling! Plus, since the light illuminates the tissue, only the target area will be treated.  If you're considering Aveli, it's important to know that there are a few requirements to keep in mind. To be a good candidate, you must:
  • No previous liposuction to the same area
  • BMI under 25
  • Minimal skin laxity
If you have skin laxity, this may not be the best treatment for you. However, it may be combined with other treatments for the best results. Remember, while there are no age restrictions, most patients who benefit from this treatment fall between the ages of 25 to 45 years old.
The procedures are similar but with a few key differences.  Cellfina used to be the most advanced deceive for cellulite until Aveli came along. While both procedures address cellulite, the main difference between Aveli cellulite treatment and Cellifina comes down to a matter of precision. To put it simply, Aveli has the edge of precision, and many providers prefer to use it.  The light that guides the Aveli device allows for more precise navigation to the areas that need treatment and leaves healthy tissue intact, so only the trouble area is addressed. The light allows doctors to see exactly which bands are problematic and not only leave healthy tissue alone but ensure that all problematic fibrous bands are severed.  Celifina does not allow for such precision, and doctors are virtually blind as they go in to treat the cellulite areas. Unfortunately, this means that some healthy tissue is damaged in the process and has a higher probability of overtreatment. Ultimately, this can impact recovery times and lead to patients having unnecessary bruising and swelling.  Likewise, undertreatment is also an issue with Celfina since the provider can't see the tissue up close. This means that patients may quire additional costly treatments. Aveli ensures that patients get the balance of complete treatment without the worry of under or overtreatment.  Aveli is considered the cutting-edge treatment for cellulite reduction, and providers prefer it over other options since it saves patients time and money. 
Aveli cellulite treatments can vary, but most treatments take only one hour to complete. However, if patients are being treated in larger areas, it will likely take a bit longer. Your doctor can give you a better time estimate once you have a complete consultation and evaluation of the treatment area.  When it comes to considering how long the treatment will last, another concern is about the length of recovery. This procedure has very little downtown, and the results are long-lasting. Patients can usually see initial results within 14 days after treatment once the bruising and swelling have gone down and more visible results after 30 days. However, full results should be seen at the 90-day mark as there should be zero side effects from the procedure. Since the treatment is relatively new, there need to be more studies to determine how often someone may need to go in for additional treatments in the future.  Aveli cellulite treatment is considered a minimally invasive treatment with a precision that requires very little downtime. In fact, most people can return to their regular activities after a day or two of rest. However, your doctor may request that you avoid certain lower-body workouts to allow your body full time to recover. Some side effects you may experience during recovery include:
  • Bruising
  • Swelling 
  • Small amounts of fluid secretion
These symptoms are described as mild and typically subside in a day or two. Should your symptoms last longer, reach out for additional guidance.
Cellulite impacts the lives of 90% of women, and many experts believe that it's a result of hormones and has nothing to do with a person's size or shape.
